Salary: £36,922 - £39,880 - Grade 11
Working Pattern: Full-time (37 hours per week)
Contract Length: Permanent
 Location: Horsham. Flexible working hours offered. Currently home based.

We are looking for a Senior Practitioner Occupational Therapist to provide specialist advice, knowledge and skill at the front door of Adult Social Care, Adult Carepoint 2 (ACP2).

We are looking for resilient and adaptable individuals to join our ACP2 team. You will be working alongside a multidisciplinary team including Social Workers, Occupational Therapists and Assessment Officers (AO). The ACP2 team key function is to undertake initial telephone Care Act Assessment with customers, their carers or health colleagues. The ACP2 team begin, and in some circumstances complete, a statutory needs assessment with customers who are approaching WSCC for advice, guidance and support.

You will thrive in a fast-paced environment, work well under pressure and be extremely organised, ensuring that key developments are communicated to colleagues and that business change is successfully implemented.

We are looking for a candidate who will be able to support and develop team performance, manage incoming demand through allocation and implement practice improvements with the team. You'll need to be committed to evidence based practice and continuing professional development, ensuring that your practice is up to date. You will also be enthusiastic about mentoring and developing others.

Experience and Skills

Key Skills:

  • Expertise in applying a risk enablement approach and using evidence-based practice.
  • Expertise in completion of strengths-based assessment (as directed by the Care Act 2014).
  • Expertise in developing a person-centred care and support plan with clearly defined customer outcomes including advice and equipment
  • Excellent communication and interpersonal skills, including the ability to use a variety of information technologies.
  • Excellent supervision, coaching and mentoring skills.
  • Expertise in prioritising and organising own work and work of others to meet agreed performance expectations.
Qualifications and/or experience:

  • Diploma /BSc Occupational Therapy or equivalent Occupational Therapy qualification.
  • Registration with Health and Care Professions Council (HCPC).
  • Relevant post qualifying experience at a senior level with examples of applying Occupational Therapy practice to complex issues.
  • Experience of supervising, coaching and mentoring of staff and students.
