This is an opportunity to join a new Older People’s Team working across Adult Health and Social Care.
Your workload will include outcome-focussed assessment, support planning, care management, review of complex care packages and contributing to the protection of adults at risk of harm.
You will have a working knowledge and understanding of Adult Support and Protection guidelines, legislation and processes.
You will have experience of working in a relevant setting across Adult and Older People Services and have a working knowledge of professional practice models and standards.
The post requires good organisational skills and the ability to manage and prioritise a significant case load. You will be skilled in assessing and analysing situations in order to make clear and sound decisions and be a good problem solver.
The post involves partnership working with service users, carers, family members, health and social work colleagues as well as other professionals and external agencies as required.
The essential qualifications for this post are a degree or Diploma in Social Work. Registration with the SSSC as a Social Worker is essential. Training as a council officer is highly desirable. The ability to travel throughout Midlothian is a requirement.
