We have a brilliant opportunity for you to join our Transforming Care team, supporting our customers in their own homes across Kent, who have complex needs including learning disabilities, autism, a diagnosis of mental health conditions, personality disorders, communication differences, alongside ongoing physical health conditions. They may also have a forensic background and support needs related to their behaviour.
You’ll work alongside our fantastic team including our Clinical Psychologist, Clinical Registered Manager and Support Workers/Team Leaders in delivering safe care, that improves quality of life and maximises customers independence. You’ll support our team in working with our customers with presenting difficulties to recognise their strengths, identify the things they want and aspire to do in their lives and embrace their future potential. This will include the assessment and management of a range of complex occupational therapy needs, diverse presentations and a range of mental health conditions, cognitive and/or learning disabilities. You’ll also identify where skills development is required so our customers can achieve their goals through everyday activities. You’ll work closely with our support team to deliver personalised interventions, skills development plans and behaviour support plans as well as well as regular reviews.
Our team are super passionate about supporting customers to reach their goals and celebrate their achievements. You will be supported by our wider New Models of Care Clinical Team who come from all backgrounds including Nurses, behavioural experts and a brilliant group of OT’s who will provide guidance and support. You’ll report initially to Karen, our Head of Transforming Care who has a wealth of experience from her clinical and operational roles working across the NHS, Social Care and private sector. She’s also a dual registered nurse and qualified in Applied Behavioural Analysis. Outside of work you’ll find her keeping fit in dance aerobics classes, singing in a choir (which she’s just recently joined) and discussing the latest book as a member of a book club (we wonder what they’re reading at the moment?).

A bit about you
You’ll have experience of working with people who have complex needs including learning disabilities, mental health and autistic spectrum disorder offering occupational therapy-based interventions. You’ll hold relevant professional qualifications including HCPC Occupational Therapist. You’ll really impress us with your ability to understand where our customers are in their lives, using evidence-based practice, care delivery and current occupational therapy-based interventions.
You’ll support in the development of our exciting new LIFE support practice model (Living Independently and Feeling Enabled) contributing to individualised plans using your OT specialism delivering in a safe, effective, caring, responsive and well-led way. We’ll tell you all about it when we meet you!
You must be able to drive and have access to a vehicle for work as you'll be required to travel and work throughout the county of Kent.
An Enhanced Check under the Disclosure and Barring Service is required for this role and will be paid for by us.
