An exciting opportunity has arisen for a Senior Social Worker within the Young Adult Disability (YAD) Team, supporting the transition of individuals from children to adult services. The role requires strong partnership working with a committed, focused, dynamic and forward-thinking leadership approach. The successful candidate will be responsible for and provide direct supervision to a team of Social Workers and will co-ordinate the ongoing management of transition for young people with a disability between the ages of 14 – 25, providing a single point of contact to hold discussions and plan for the next steps in the persons life.

The team works alongside the Children Affected by Disability (CAD) Team and are ordinarily co-located to ensure a cross departmental approach. Their main aim is to plan with young people to explore community resources, make connections within communities, explore informal networks, and plan for formalised funded support when this is appropriate. This support could be for day opportunities or support with planning a move from the family home. The team also undertake all other case management tasks which may be required and include adult protection.

The transition period can be a challenging time for young people and their families and the role requires the successful candidate to work under pressure, forward plan and establish productive working relationships with all key stakeholders.
