We are looking for a full time experienced social worker to join our team at the Great Western Hospital in Swindon.
We currently have an exciting opportunity for a full time Experienced Social Worker to join the Wiltshire Adult Social Care Team at Great Western Hospital in Swindon. The forward thinking and enthusiastic team work together with hospital staff to achieve safe and timely transfers of care.
Our hospital teams are based in three acute hospitals and provide advice, assessment and discharge planning for people. All of these teams work closely with health colleagues to promote an effective multi-disciplinary approach
We’re exploring innovative front door of hospital approaches to ensure the correct discharge pathway is instigated as soon as possible.
You will be an experienced social worker with at least 2 years practising experience. You will have enough experience to confidently offer peer supervision and support to our social workers and trained customer coordinators. In addition, it is important you are an organised professional that has an unrivalled knowledge of all relevant legislation. You will be an experienced social worker within adult social care and ideally you will have a background within a hospital environment.

About the role
Our team is a small and enthusiastic team who work together with the hospital to achieve safe and timely transfers of care.
The experienced social worker is expected to hold a caseload and undertake complex assessments to facilitate discharge, as well as undertaking the Investigating Officer role in safeguarding cases. Knowledge of the Mental Capacity Act 2005 and the Care Act 2014 is essential and you will have experience in applying these legislative frameworks, as well as supporting others to apply the principles of the Act(s).
